How the service works is very simple.
If you are just wanting a one time visit, just fill out the application and you will be contacted for further information about the upcoming visit. Are you a returning patient and want further services. Just fill out the follow-up office visit form and skip part one unless your contact information or insurance information has changed.
Enjoyed your first visit and want to have more services. Once you joined you'll be sent an email welcoming you to the Be Healthy We Care Society and Family.
